Job Title: Trinity House Case Manager

General Summary
The Trinity House Case Manager will provide life skills programming and case management to Trinity House residents and manage all housing inquiries and intakes. All work is accomplished in accordance with Vida’s Code of Christian Conduct, Statement of Faith, policies, and procedures.
Primary Responsibilities
- Manage resident admissions process: Conduct thorough intakes and assessments for new residents, collaborate with the housing team to review and staff housing applications, facilitate orientations, and ensure smooth transitions into the home.
- Provide intensive, individualized mentoring and support to Trinity House residents through:
- Scheduling and conducting regular (weekly) one-on-one appointments to build trust, review progress, and address emerging needs.
- Assessing residents’ holistic needs (prenatal health, emotional well-being, housing stability, parenting readiness, education/employment, etc.) and co-developing strengths-based, individualized case plans with clear, measurable goals.
- Maintaining accurate, up-to-date, and comprehensive resident files (paper and electronic), ensuring all documentation complies with confidentiality policies.
- Support residents’ prenatal and postnatal health by coordinating medical appointments (OB/GYN, prenatal classes, ultrasounds, etc.), arranging transportation as needed, connecting to resources like WIC, Medicaid, childbirth education, doula/midwife services, and monitoring adherence to health recommendations.
- Assist residents in pursuing education, employment, or career goals through goal-setting, resume support, job search assistance, referral to training programs, and tracking progress toward self-sufficiency.
- Serve as a primary liaison and advocate: Refer residents to appropriate external community services (e.g., mental health counseling, domestic violence support, substance use programs if relevant, legal aid) and maintain collaborative relationships with outside agencies.
- Collect, maintain, and report required resident data, progress notes, and outcomes in a timely manner to support program evaluation and funding compliance.
- Participate actively in resident corrective action discussions, providing trauma-informed input and supporting fair, consistent implementation of house rules and behavior plans.
- Respond to resident crises (emotional, medical, relational) with de-escalation, safety planning, and appropriate referrals.
- Adhere to all Vida policies and procedures, including strict confidentiality, trauma-informed practices, and resident rights.
- Participate in the on-call rotation to provide after-hours support to residential staff and respond to resident emergencies or urgent questions, including one weeknight per week and every fourth weekend.
Secondary Responsibilities
- Attend and contribute to regular Trinity House team meetings, staff meetings, and manager meetings to ensure coordinated program delivery and smooth day-to-day operations.
- Support implementation of the program’s case management and overall management plan, including contributing to continuous quality improvement.
- Participate in agency-wide activities such as annual fundraising events and other assigned projects as directed by the Executive Director.
- Perform other duties as required to support the mission of Trinity House and the well-being of residents.
Qualifications
- Bachelor’s degree or job-related equivalent experience in human resources, social work, psychology, or related service field
- Requires a minimum of 1 year of hands on experience working in case management in human services, maternity health, shelter or child protection programs.
- Expresses full agreement with organization’s Statement of Faith and Mission Statement
- Ability to comply with the policies and procedures of the organization
- Dependable, stable, and capable of following through on commitments
- Knowledge of Microsoft Office including Excel, Word, and SharePoint
- Excellent interpersonal skills
- Excellent oral communication skills
- Ability to write thorough and comprehensive case notes
